Severe infections are a major cause of death worldwide. Within the comprehensive management of the patient in severe sepsis or septic shock, early administration of an appropriate antibiotic regimen is one of the key strategies for survival and decrease of morbidity and mortality. This review presents the antibiotic treatment of severe intra-abdominal infections and serious infections of the skin and soft tissues. General surgeons who treat serious infections, need to know not only the clinical manifestations of each process, but also its epidemiological characteristics, local bacterial prevalence, resistance patterns and pharmacology of antibiotics available to make the best decision. Guidelines and systematic reviews are fundamental tools for the prophylaxis and management of surgical severe infections.
