Características Inmuno Histoquímicas del Cáncer de Mama en varones

Breast cancer is an unusual pathology in males, it accounts for less than 1% of breast malignancies and less than 1% of malignant diseases in males. As in women it is a heterogeneous disease and its classical assessment is based on clinical parameters as the TNM system and histopathology which allow to group patients for treatment alternatives and survival prognosis as we can be led to over or under estimate a treatment choice. We pretend to sub-classify breast cancer in males based on immunohistochemical studies which are closely related to prognosis and are predictors of response to treatment.
We reviewed clinical records of male patients with breast cancer at the INEN, 33 were found but only 27 selected as they had all the immunohistochemical studies. Four immunohistochemical studies based on hormonal receptors (estrogen, progesterone) and/or Her2neu. Luminal A and B, Her2neu and triple negative were studied if there were diffrences between them and their relation with clinical, histopathological and immunohistochemical parameters of known prognostic value. This study found 22 cases (81.47%) related to the Luminal immunophenotype, 13(48.14%) were Luminal A and 9(33.33%) were Luminal B, 3 cases (11.11%) were triple negative and 2(7.42%) were Her2neu. Infiltrating carcinoma accounted for 92.6%. Luminal A tumors had a
significant androgenic receptor expression. Intermedíate nuclear grade was found in 63% and increased nuclear grade in 29.6%. Lymph node involvement was found in 55.6% and in the Luminal B sub-type it was 77.8%(7/9). The number of patients with more than 10 positive nodes was greater for the Luminal B su-type (33.3%) than for Luminal A (7.7%). Surgical treatment followed by hormonal therapy was performed in 78.8%, more frequently in Luminal A tan B. We found that Luminal B was realted to high nuclear grade and higher lymph node involvement.
