Peptic ulcer disease is the most common cause of stomach and duodenal perforation. We report the case of a 78-year-old male with past medical history of duodenal peptic ulcer disease presenting with acute abdominal pain and severe abdominal distention. Abdominal CT scan showed a injury in the first portion of duodenum`s wall and other radiological alterations that suggested the possibility of duodenal perforation. The appearance of pneumoperitoneum in de abdominal CT scan plus confirmed the diagnosis. With a healthy diet and postoperative follow-up, the patient remains asymptomatic.
