In may 2022, more than 3,000 monkeypox virus infections were reported. Brazil, Peru, Colombia and Mexico were the countries with the most confirmed cases. Proctitis and perianal sepsis can be surgical management with surgical cleansing bowel resection, and use of stomas. Methodology: Observational, retrospective study that describes the use of diverting colostomies in patients with monkeypox with proctitis and perianal sepsis from January 2022 to January 2023. Results: 23 patients were included. The mean age was 33.3 years. 17 (73.9%) had a diagnosis of human immunodeficiency virus (HIV) infection. The average number of days of hospitalization was 17 days. 9 (39.1%) patients had proctitis, a diverting colostomy was required in 3 of them. Mortality was 8.3% (2/23). Conclusions: The use of diverting colostomies is an alternative to consider in the management of proctitis and perianal sepsis in patients with monkeypox
