Abstract

The management of splenic trauma has changed over the years, currently medical approach is the first line of treatment, which includes hemodynamic support and embolization by interventional radiology. Surgical approach is performed when medical management fails, various studies mention the benefits of a laparoscopic approach in this type of case. A 34-year-old male patient who underwent blunt abdominal trauma, was admitted to the emergency room hemodinamically unstable, with a drop in hemoglobin, stabilized with fluid therapy, a CT scan was performed which found: diffuse laceration of the splenic parenchyma, with involvement of vascular hilum and dot sign (injury to the splenic hilum). The patient underwent exploratory laparoscopy + splenectomy, discharged on the fifth postoperative period. First report of laparoscopic splenectomy in grade V splenic trauma, demonstrating that with trained personnel a minimally invasive, safe and fast recovery surgery can be performed.
