Abstract

Obturator hernia represents only 0.05-1.4% of all hernias of the abdominal wall and is responsible for 0.2-1.6% of the causes of intestinal obstruction. The coexistence of obturator, femoral and inguinal hernia is even rarer. We present the case of an 84-year-old multiparous woman, admitted to emergency with a 1-day illness, characterized by abdominal pain in the pelvic region that radiates to the anterior aspect of the right thigh, associated with vomiting and absence of bowel movements and flatus. She was diagnosed with obturator and femoral hernias, with signs of incarceration that required emergency surgical treatment. Finding in surgery, an intestinal perforation due to a complicated obturator hernia as well as a femoral hernia was evidenced and, incidentally, an ipsilateral inguinal hernia was found. An intestinal resection with anastomosis was performed along
with surgical treatment of hernia defects. The patient had an uneventful recovery and was discharged on the tenth postoperative day.
