Abdominal midline defects (umbilical, epigastric, incisional hernias and diastasis recti) require surgical correction due to loss of the abdominal press and anterior protrusion of the abdominal wall. Complete surgical repair of these defects seeks to restore proper dynamics, press and abdominal aesthetics.
We present the surgical technique of complete repair of the abdominal midline by totally endoscopic approach with plication of diastasis of recti, closure of associated hernias and prefascial mesh placement. We describe the technical steps and first results in 18 patients, with a postoperative follow-up of12 months. There was a case of recurrence in the upper segment and a chronic seroma that required reoperations.
Endoscopic repair of the midline is presented as a safe, reproducible technique with adequate results in the dynamics and aesthetics of the anterior abdominal wall.
