Abstract

Chronic appendicitis is a rare pathology with an incidence of 1.5% of all cases . It is difficult to diagnose due to the atypical presentation of the disease and requires high clinical suspicion.

Report. Female 66 years old patient who complains of hypogastric pain for one month, of evolution under medication with mild response to treatment . Assesment concluded in Irritable Bowel Syndrome for which she was treated; she became pyrexial and a CT scan showed a pelvic collection with no evidence of the appendix. Colonoscopy reveals pus through the appendicular orifice. She underwent laparotomy with resection of the appendix after which she made a good recovery.

Chronic appendicitis is a disease of difficult diagnosis due to atypical signs, in these cases good clinical judgment is required and adequate use of diagnostic tools for an adequate approach.
