The list of techniques for inguinal hernia repair is numerous. Stoppa’s technique includes the placement of a large mesh in order to reinforce the visceral sac. The aim of this review is to assess Stoppa’s technique for the treatment of complex groin hernias. During a 5 year period 55 patients with bilateral, massive and recurrent hernias were selected. Polypropylene mesh was used and no drains were left in place; there were minor complications and two recurrences. There were no infections, hematomas, dehiscence, nor mortality. Stoppa’s hernioplasty is safe and efficient for complex hernias in the groin. Careful selection and meticulous technique are important for this intervention that can be considered as fairly complex within its category. This procedure proved to be useful in the recognition of the posterior anatomy of the groin, a basic step to start laparoscopic techniques.
